Ulthera Prime vs Thread Lift: Non-Surgical Facial Lifting Compared — a guide for visitors in Myeongdong, Seoul.
Quick answer

Ulthera Prime uses focused ultrasound with real-time imaging to heat the deep SMAS layer, producing a gradual lift over two to three months with no needles and almost no downtime; results commonly last about a year. A thread lift uses dissolvable PDO threads placed under the skin to physically reposition sagging tissue, giving a visible lift straight away but with 3–7 days of swelling or bruising; results commonly last 12–18 months. At Dernia Clinic Myeongdong, Ulthera Prime starts from ₩390,000 (100 lines) and a thread lift from ₩690,000 per area, excluding 10% VAT.

Ulthera Prime vs thread lift: the key difference

Both are non-surgical ways to lift a softening jawline, early jowls or flattened cheeks, but they work in opposite ways. Ulthera Prime uses focused ultrasound to heat the deep support layer of the face (the SMAS). Nothing is inserted, and the lift develops gradually as collagen rebuilds. A thread lift uses dissolvable PDO threads placed under the skin through small entry points. Barbed or cog threads hold and reposition tissue, so the lift is visible straight away, and collagen forms around the threads as they dissolve.

The trade-off is simple: Ulthera is gentler with almost no downtime but gives a more subtle, gradual change; a thread lift gives a more immediate, mechanical lift but involves needles, local anesthetic and several days of recovery. For background, see Korean lifting treatments explained.

How does Ulthera Prime work?

Ulthera Prime delivers rows of tiny heating points at set depths, commonly around 4.5 mm for the SMAS, 3.0 mm for the deep dermis and 1.5 mm for more superficial skin. Real-time ultrasound imaging lets the doctor see tissue layers on screen before placing each line, which helps deliver energy precisely. The heat contracts tissue and triggers new collagen, with the main lift at two to three months. See the Ulthera Prime page.

How does a thread lift work?

A thread lift uses fine, dissolvable threads made of PDO (polydioxanone). After local anesthetic, the doctor inserts the threads under the skin with a fine needle or cannula along planned vectors. Barbed or cog threads grip the tissue and lift it into a higher position; finer threads mainly improve firmness. Over the following months the threads dissolve, while the collagen that forms around them helps maintain support. The number and type of threads depend on your anatomy. See the thread lift page.

Side-by-side comparison

Factor Ulthera Prime Thread lift
Method Focused ultrasound, nothing inserted Dissolvable PDO threads under the skin
Anesthesia Numbing cream Local anesthetic at entry points
When you see it Gradual; main lift at 2–3 months Immediate, then settles over weeks
Downtime None to minimal 3–7 days of swelling or bruising
Results last About a year or longer About 12–18 months
Strength of lift Subtle to moderate Moderate, more mechanical
Main risks Temporary tenderness, numbness; rare nerve effects Bruising, dimpling, asymmetry, infection, thread visibility

Which is better for facial lifting?

The right choice depends on how much lift you need, how much downtime you can accept and your comfort with needles.

  • Choose Ulthera Prime if you have early to mild sagging, want no visible recovery during your trip and are happy with a natural, gradual result.
  • Choose a thread lift if you have moderate sagging of the mid-cheek or jowls and want a more noticeable, immediate repositioning, and you can plan several quiet days.
  • Combine them in some cases, often on separate visits: threads for mechanical lift, Ulthera for deeper tightening. Your doctor will advise on timing.

People with heavy sagging or significant excess skin should know that neither option replaces a surgical facelift. Typical timelines are on the Ulthera Prime results and thread lift results pages.

Who is a good candidate?

Usually a good fit

  • Ulthera Prime: early jowls, softening jawline, lower brows, no-downtime preference
  • Thread lift: moderate mid-cheek or jowl descent, reasonable skin thickness
  • Adults who want a lift without surgery
  • Visitors with realistic expectations about non-surgical results

Talk to the doctor first

  • Heavy sagging or excess skin, where surgery may suit better
  • Very thin skin or a very thin face
  • Blood thinners, bleeding disorders or keloid tendency (for threads)
  • Pregnancy, active infection, autoimmune conditions or recent filler in the area

What happens on treatment day

  1. English consultation. A licensed doctor assesses your sagging and skin, explains which option suits you and agrees the plan and price before anything starts.
  2. Preparation. For Ulthera, numbing cream is applied for 30–40 minutes. For threads, the skin is disinfected, entry points and vectors are marked and local anesthetic is injected.
  3. Treatment. Ulthera lines are placed at planned depths using imaging. Threads are inserted along the marked vectors and the tissue is gently adjusted for symmetry.
  4. Aftercare. You receive written aftercare in English. After threads, you may be asked to avoid wide mouth opening and pressure on the face for a period.

Recovery and aftercare

After Ulthera Prime, most people have mild redness for a few hours and some tenderness along the jaw for a few days. You can usually return to sightseeing the same day.

After a thread lift, expect swelling, bruising and some tightness for about 3–7 days. Small puckers or dimples near entry points are common early on and usually settle as swelling resolves. You will typically be advised to sleep on your back, avoid wide yawning, chewing hard foods, dental work, facial massage, saunas and intense exercise for a period specified by your doctor.

Is it safe? Risks and side effects

Ulthera Prime is considered low risk in trained hands. Uncommon effects include small bruises, welts in a line pattern or temporary numbness; rarely, energy placed too deep in thin areas can cause temporary nerve weakness or unwanted fat loss.

Thread lifts carry more risk because something is placed under the skin. Common effects are bruising, swelling and tenderness. Less common risks include dimpling, asymmetry, threads that can be felt or seen, thread migration or extrusion, infection and temporary nerve irritation. Some problems may need threads to be adjusted or removed. A doctor experienced with facial anatomy and sterile technique matters. Contact the clinic promptly if you notice increasing pain, redness, warmth, discharge, fever or a thread poking through the skin.

Planning a lift around your Seoul trip

  • Book ahead. Send your dates and photos through the consultation form so a doctor can suggest a plan.
  • Avoid your arrival day. Schedule treatment for day two or later.
  • Thread lift timing. Have threads early in your trip so the doctor can check you before you fly, ideally with at least five to seven days before departure.
  • Getting here. Dernia is near Myeongdong Station (Line 4) and Euljiro 1-ga Station (Line 2).

Flying home? After Ulthera, most people can fly the next day. After a thread lift, flying is usually possible once the doctor has checked you, but swelling may last a few days, so pack a soft pillow and avoid pressing on your face. See our aftercare guide.

Frequently asked questions

Which gives a bigger lift, Ulthera or a thread lift?

A thread lift usually gives a more immediate and noticeable mechanical lift. Ulthera Prime gives a subtler, gradual lift with almost no downtime.

How long does a thread lift last?

Results commonly last about 12–18 months as collagen forms around the dissolving threads. Ulthera Prime commonly lasts about a year.

Is a thread lift painful?

Local anesthetic is used, so most people feel pressure and tugging rather than sharp pain. Soreness and tightness are common for several days.

What are the risks of a thread lift?

Bruising and swelling are common. Less common risks include dimpling, asymmetry, visible or palpable threads, thread extrusion and infection.

How much do they cost at Dernia?

Ulthera Prime starts from ₩390,000 for 100 lines and a thread lift from ₩690,000 per area, excluding 10% VAT.

Can I combine Ulthera and threads?

Sometimes, usually on separate visits. Your doctor will advise on the order and spacing.
