How cheek filler restores lost volume and supports the midface, what a natural Korean-style result looks like, how long it lasts and how to plan it safely during a trip to Seoul.
Cheek filler uses hyaluronic acid (HA) gel to restore volume in the midface, either over the cheekbone for lift and contour or in the front of the cheek for a softer, fuller look. Restoring cheek support can also soften the look of smile lines and tired under-eyes. Treatment takes 20–30 minutes, most people need 1–2cc, swelling settles in a few days and results usually last about 12–18 months. At Dernia Clinic Myeongdong in Seoul, Korean HA filler starts from ₩150,000 for 1cc (KRW, excluding 10% VAT).
Prices are in Korean won, exclude 10% VAT and are confirmed by a doctor at consultation.
What is cheek filler?
Cheek filler is the injection of hyaluronic acid (HA) gel into the midface to restore or enhance volume. The cheeks are supported by fat pads and bone that change with age: fat pads thin and slide downward, and the bone of the midface slowly loses projection. The result is a flatter cheek, a more visible tear trough, deeper nasolabial folds and sometimes early jowls.
By replacing support in the right places, cheek filler can make the face look fresher and more balanced, often with less filler than people expect. It is one of the most versatile filler treatments because it influences the areas around it as well. Read more on our dermal filler page and the cheek area page.
Volume restoration vs contouring: two goals
Restoring lost volume
For people in their late 30s and beyond, cheek filler is usually about replacing what has been lost. Filler is placed deep on the bone and in the fat compartments to rebuild support, which can soften the look of nasolabial folds and the hollow between the cheek and lower eyelid. See facial volume loss.
Balancing facial shape
Younger visitors often want a softer front cheek, sometimes called "apple cheeks" in Korea, which gives a youthful, rounded look from the front. Others want to smooth a flat area beside a prominent cheekbone so the face looks less angular. Korean aesthetics usually favor a smooth curve from the temple to the cheek to the jaw, rather than very high, sharp cheekbones.
The right approach depends on your bone structure. Adding volume to an already wide cheekbone can make the face look broader, so a careful doctor considers the whole face before deciding where filler goes.
Who is a good candidate for cheek filler?
Usually a good fit
- Flattening or hollowing in the midface
- Tired look from loss of cheek support
- Mild smile lines linked to midface volume loss
- People wanting a soft, rounded front cheek
Talk to the doctor first
- Significant sagging, where lifting may be more useful than volume
- A naturally wide or prominent cheekbone
- Previous permanent filler or fat grafting in the cheeks
- Pregnancy, breastfeeding or autoimmune disease
If skin laxity is the main issue, filler alone can make the face look heavy. Lifting with ultrasound or RF, or a collagen stimulator, may be a better first step. Compare options in dermal filler vs Sculptra.
What happens on treatment day
- English consultation. The doctor examines your face from several angles, at rest and smiling, and agrees the product, volume and price with you.
- Photos and marking. Reference photos are taken and the injection points are marked.
- Numbing. Topical cream is applied, and most cheek fillers contain lidocaine for comfort.
- Injection. Filler is placed with a needle at the bone or with a blunt cannula in the fat layer, in small amounts, checking symmetry throughout.
- Review. You see the result in a mirror and receive written aftercare in English.
Cheek filler results timeline
| When | What you may notice |
|---|---|
| Day 0 | Immediate lift and volume, plus some swelling |
| Days 1–3 | Swelling peaks then eases; tenderness when smiling or chewing |
| Week 2 | Filler settles into the tissue; this is your true result |
| Months 12–18 | Gradual softening; a smaller top-up maintains the shape |
Duration depends on product, depth, volume and metabolism. Deep placement on the bone with a firmer gel tends to last longer than softer, superficial filler. See dermal filler results.
Is cheek filler safe? Risks and side effects
The cheeks are generally a lower-risk filler area, but serious complications are possible with any filler.
- Common: swelling, tenderness, bruising and mild asymmetry while swelling settles.
- Less common: lumps, visible product, or filler spreading under the eye. HA can usually be dissolved.
- Uncommon: infection or delayed nodules.
- Rare but serious: vascular occlusion. The midface has important vessels, so severe pain, blanching or a mottled, dusky patch of skin need urgent treatment.
- Over-filling: too much cheek volume can look puffy or unnatural, sometimes called "pillow face". A conservative, staged approach avoids this.

For gradual, longer-lasting volume, some people choose a collagen stimulator instead. Lenisna and Sculptra build volume over months rather than instantly.
Cheek filler vs lifting: which do you need?
Many visitors arrive asking for cheek filler when the real concern is sagging, or asking for lifting when the main issue is lost volume. The two problems look similar in photos but need different tools.
- Volume loss shows as flatness, hollows and shadows. Filler or a collagen stimulator replaces the missing support.
- Laxity shows as skin and soft tissue drifting downward toward the jaw. Ultrasound or radiofrequency lifting, or threads, address that better than volume.
- Both together are common from the 40s onward. A combined plan often uses less filler, because lifting first reduces how much volume is needed.

Recovery and aftercare
- First 24–48 hours: avoid pressing on your cheeks, wearing heavy glasses that rest on them and sleeping face-down.
- First 2–3 days: skip saunas, jjimjilbang, intense exercise and alcohol.
- First two weeks: avoid facial massage, dental work and heat-based treatments on the cheeks unless the doctor approves.
- Chewing: mild tenderness when eating is normal for a few days.
Flying home? Most people can fly one to two days after cheek filler. If possible, stay three to five days so the doctor can check the result once swelling settles. Read flying after Botox and fillers.

Frequently asked questions
How much cheek filler do I need?
Most people need about 1–2cc across both cheeks. More may be needed for significant volume loss, often spread over more than one visit.
How long does cheek filler last?
Usually about 12–18 months, depending on the product, depth and your metabolism.
Will cheek filler make my face look wider?
It can if filler is added to an already wide cheekbone. A careful doctor places filler to restore balance, not width.
Can cheek filler help smile lines?
Often, yes. Restoring midface support can soften the look of nasolabial folds, sometimes better than filling the fold directly.
How much is cheek filler in Seoul?
At Dernia Clinic Myeongdong, Korean HA filler is ₩150,000 for 1cc or ₩280,000 for 2cc, and imported brands cost ₩290,000–₩350,000 per cc, excluding 10% VAT.
