Filler is one of the most common ways to soften smile lines, but it is not always the best one. Here is an honest look at what nasolabial fold filler can achieve, where it falls short and what else may help.
Nasolabial fold filler places hyaluronic acid (HA) gel along or beneath the lines running from the nose to the corners of the mouth to soften their depth. It works well for moderate folds, gives an immediate result and usually lasts about 6–12 months. Its main limitation is that folds are often caused by midface volume loss or sagging, so filling the line alone can look heavy. At Dernia Clinic Myeongdong in Seoul, Korean HA filler starts from ₩150,000 per cc (KRW, excluding 10% VAT).
Prices are in Korean won, exclude 10% VAT and are confirmed by a doctor at consultation.
What are nasolabial folds?
Nasolabial folds, often called smile lines or laugh lines, run from the sides of the nose to the corners of the mouth. Everyone has them to some degree because they mark the border between the cheek and the upper lip. They become deeper with age for several reasons: the cheek fat pads lose volume and slide down, the bone around the nose recedes, and skin loses collagen and elasticity.
Understanding the cause matters, because it changes which treatment works best. Learn more on the nasolabial folds concern page.
How does nasolabial fold filler work?
HA filler is injected along the fold, beneath it near the bone at the top of the fold, or both. It lifts the crease from underneath so the shadow becomes softer. Different techniques are used depending on depth: a firmer gel may support the base near the nose, while a softer gel smooths the line itself so it moves naturally when you smile.
Results are visible right away and settle over about two weeks. Because the area moves a great deal when you talk and smile, filler here tends to break down sooner than in the chin or cheekbones. See our dermal filler page.
Benefits of nasolabial fold filler
- Immediate softening. You see the change the same day.
- Quick and adjustable. Small amounts can be added in stages.
- Reversible. HA can be dissolved with hyaluronidase if needed.
- Useful for a deep base. Support near the side of the nose can help where bone has receded.
- Short recovery. Most people have mild swelling for a day or two.
Limitations: when filler is not the best answer
Many people, and some clinics, treat the fold as the problem. Often it is a symptom. These are the most common limitations to understand before booking.
1. Folds caused by cheek volume loss
If the cheek above has flattened, the fold deepens because the tissue above it has descended. Restoring cheek support with midface volume can soften the fold more naturally than filling the line itself.
2. Folds caused by sagging
When laxity is the main cause, adding filler to the fold can make the lower face look heavy or puffy. Lifting with ultrasound, RF or threads may be more useful. See sagging skin treatments.
3. Over-filling
Filling a fold completely flat looks unnatural, because a soft fold is part of a normal face. Over-filled folds can create a ridge or a puffy look when smiling. A good result softens the line rather than erasing it.
4. Shorter duration
Constant movement around the mouth means results often last about 6–12 months, shorter than in less mobile areas.
Usually a good fit
- Moderate folds with reasonably good cheek volume
- A deep hollow at the top of the fold beside the nose
- People who want a quick, reversible improvement
- Those already planning cheek support or lifting
Talk to the doctor first
- Folds mainly caused by sagging or heavy jowls
- Very full lower face or prominent cheeks
- Previous permanent filler in the area
- Pregnancy, breastfeeding or autoimmune conditions
Nasolabial fold treatment options compared
| Option | Best when the cause is | Onset | Notes |
|---|---|---|---|
| Filler in the fold | A deep crease with good cheek support | Immediate | Lasts about 6–12 months; avoid over-filling |
| Cheek filler | Midface volume loss | Immediate | Indirectly softens the fold; often lasts longer |
| Collagen stimulator | Gradual volume loss | 2–3 months | Not dissolvable; planned over sessions |
| Lifting device or thread lift | Sagging tissue | Weeks to months | Treats laxity, not volume |
Many people get the most natural result from a combination, for example light lifting plus a small amount of filler. Our guide to Korean lifting treatments explains the device options.
What happens on treatment day
- English consultation. The doctor looks at your face at rest and smiling, checks cheek volume and skin laxity, and explains whether the fold itself, the cheek above it or both should be treated. Product, volume and price are agreed before anything starts.
- Photos and numbing. Reference photos are taken and numbing cream is applied. Most HA fillers also contain lidocaine.
- Injection. Filler is placed with a fine needle or a blunt cannula, in small amounts, checking the shape as you smile.
- Review and aftercare. You check the result in a mirror and receive written aftercare in English, including which warning signs need an immediate call to the clinic.
Results timeline
The fold looks softer straight away, though swelling can make it look slightly fuller for the first two or three days. By about two weeks the filler has settled and you can judge the true result. Over the following months the gel gradually breaks down; most people notice the fold returning slowly between six and twelve months. A smaller top-up usually maintains the result. Photos taken before treatment and at two weeks make it easier to see the change, because smile lines are hard to judge in a mirror. See dermal filler results.
Is nasolabial fold filler safe? Risks
The nasolabial fold sits close to the facial artery and its branches, especially near the side of the nose. This makes it a moderate-risk filler area.
- Common: swelling, tenderness and bruising for a few days.
- Less common: lumps, ridges or visible product, which HA can usually correct.
- Uncommon: infection or delayed nodules.
- Rare but serious: vascular occlusion, which can damage skin near the nose and upper lip and, very rarely, affect vision. Severe pain, blanching or dusky, mottled skin need urgent treatment with hyaluronidase.
Choose a licensed doctor who knows the vascular anatomy, uses genuine products and keeps hyaluronidase available. Read side effects and when to contact us.

Frequently asked questions
Does filler get rid of smile lines completely?
It softens them but should not erase them. A soft fold is natural, and filling it flat tends to look unnatural.
How long does nasolabial fold filler last?
Usually about 6–12 months. The area moves constantly, so filler breaks down faster than in the chin or cheekbones.
Is cheek filler better than filling the fold?
When folds are caused by midface volume loss, restoring the cheek often softens them more naturally. A doctor assesses which approach suits you.
Is nasolabial filler risky?
It is a moderate-risk area because of nearby blood vessels. Vascular occlusion is rare but serious, so choose an experienced doctor and know the warning signs.
How much is nasolabial fold filler in Seoul?
At Dernia Clinic Myeongdong, Korean HA filler is ₩150,000 for 1cc or ₩280,000 for 2cc, excluding 10% VAT.
Can lifting treat smile lines?
Lifting helps when folds are caused mainly by sagging. Many people benefit from combining lifting with a small amount of filler.
