What causes facial volume loss?

A youthful face has soft, even fullness supported by fat pads, bone and collagen. With age, the deep fat pads in the temples, cheeks and around the eyes shrink and shift downward, the bones of the face slowly recede, and the skin produces less collagen. The result is hollow temples, flatter cheeks, deeper smile lines and a tired look. Volume loss can also happen earlier with genetics, intense endurance exercise, significant weight loss, including weight loss on GLP-1 medicines, and some medical conditions.

How volume loss is assessed

At Dernia Clinic Myeongdong, your doctor looks at your face from the front, side and at angles, often in overhead light that shows shadows. Volume loss is usually mapped by area:

  • Temples: hollowing above the outer cheekbone
  • Midface: flattened cheeks that let tissue slide downward
  • Under-eyes: grooves that cast shadows; see under-eye hollows
  • Lower face: thinning around the mouth and chin

Your doctor also considers skin quality and laxity. If sagging is part of the picture, lifting may be added; see sagging skin. Your preference matters too: some people want an immediate change, others a gradual one that friends will not notice. Photos taken before treatment help you see the change over time.

Treatment options for volume loss

Dermal Filler

Dermal filler made of hyaluronic acid restores volume immediately and precisely. Your doctor chooses it for defined areas such as the cheeks, temples or chin, or when you want to see the result straight away. Different products suit different areas, from firmer gels for the cheekbones to softer ones for the temples. Results typically last 6–18 months depending on the product and area, and HA can be dissolved if needed. Korean and imported brands are available; see HA filler brands used in Korea.

Lenisna

Lenisna is a higher-concentration PDLLA collagen stimulator that builds soft, natural volume over two to three months. Your doctor chooses it for hollow temples and cheeks when you prefer a gradual change, with results that often last 12–24 months. Because it cannot be dissolved like HA, your doctor places it conservatively.

Sculptra

Sculptra is a poly-L-lactic acid (PLLA) stimulator that helps your skin build its own collagen. It is chosen for wider, more global volume loss and improves firmness too. It is usually given as a series a few weeks apart, needs regular massage afterward, and results can last up to two years. Because the change appears slowly, people tend to notice that you look well rather than that you had treatment.

Combination plans

Many plans use filler for immediate structure in key areas and a collagen stimulator for overall fullness and skin quality. Compare the options in dermal filler vs Sculptra, Lenisna vs Sculptra and Juvelook vs Sculptra. You can also explore our temple and cheek treatments.

At-home care and prevention

Keep your weight stable where possible, protect your skin from the sun and avoid smoking. After Sculptra, follow the massage routine your doctor gives you. After filler, avoid pressing on treated areas and intense exercise for a day.

Planning treatment during a trip to Seoul

Filler and Lenisna cause mild swelling for 1–3 days, so book them early in your stay. Bruising is possible, so avoid injectables right before photos or an important event. Sculptra and Lenisna build over months, so a series may mean returning or continuing at home. Book a consultation to plan around your dates.

Safety and when to see a doctor

Unexplained weight loss or facial thinning should be checked by a doctor before cosmetic treatment. Injectables are avoided during pregnancy and breastfeeding, over an active infection and with some bleeding disorders. Filler blocking a blood vessel is rare but serious, so seek help urgently for blanching, severe pain or any change in vision. Collagen stimulators cannot be dissolved, so doctors use conservative amounts.

FAQ

Frequently asked questions

Straight answers from our medical team. Still unsure? Ask a doctor or browse all patient questions .

Is filler or Sculptra better for volume loss?

Filler gives an immediate, precise result in defined areas. Sculptra builds collagen gradually for wider volume loss and lasts longer. Many people use both for different areas.

How long does Sculptra take to work?

Some fullness is visible right away from the fluid, but it fades within days. The real result builds over two to three months as collagen forms, usually after a series of sessions.

Can treatment help facial thinning after weight loss?

Yes. Filler, Lenisna and Sculptra are often used to restore volume after significant weight loss, including from GLP-1 medicines. Your doctor will plan for skin laxity too.

Will my face look overfilled?

Not if the amount is matched to your face. Doctors restore support in the right places and often treat in stages, so the result looks natural.

How long do volume treatments last?

HA filler lasts about 6–18 months, Lenisna often 12–24 months, and Sculptra up to two years after a series. Longevity varies with your metabolism and lifestyle.
