What causes sagging skin?

Skin sags when the structures that hold it up weaken. From your late 20s, collagen and elastin decline, so skin thins and loses its spring. The cheek fat pads shrink and slide down, the ligaments that anchor them loosen, and facial bone slowly recedes. Sun, smoking and big weight changes speed this up, leaving a softer jawline, deeper folds and a tired look.

How sagging is assessed

At Dernia Clinic Myeongdong, your doctor checks three layers: the skin surface, the deeper support layer called the SMAS, and volume (fat and bone). Sagging is usually graded as mild (softening seen mostly in photos), moderate (early jowls and loose skin under the chin) or advanced (heavy folds). Non-surgical lifting suits mild to moderate sagging. For advanced laxity, your doctor will honestly suggest a surgical opinion.

Treatment options for sagging skin

Most plans pair one treatment for the deep layer with one for the skin.

Ulthera Prime

Ulthera Prime lifts from the SMAS layer with imaging-guided microfocused ultrasound. Your doctor chooses it when the jawline and brow need real lift from one session.

Thermage FLX

Thermage FLX heats the deep dermis with monopolar RF to tighten and thicken skin. It is chosen for loose, crepey skin on the cheeks, eyelids and neck.

Shurink Universe

Shurink Universe is Korean HIFU that lifts at a lower price. It is chosen for early sagging or maintenance between bigger treatments.

Oligio X

Oligio X is Korean monopolar RF that tightens much like Thermage. It is chosen when you want RF tightening, including around the eyes, at a lower cost.

Volnewmer

Volnewmer is Korean RF with a water-cooled tip. It is chosen if you are sensitive to heat or plan regular sessions.

InMode Forma & Mini FX

InMode Forma & Mini FX pairs RF tightening with a handpiece for small fat pockets. It is chosen when sagging comes with fullness along the jaw.

Titanium Lifting

Titanium Lifting is a triple-wavelength diode laser that firms and adds glow with no downtime. It is chosen for mild laxity or before an event.

Onda Lifting

Onda Lifting uses microwave energy on fat and loose skin. It is chosen for a heavier lower face, where less fat makes the jaw look lifted.

Thread Lift

A thread lift repositions tissue with dissolvable PDO threads. It is chosen for moderate sagging that devices alone cannot lift; allow 3–7 days for swelling.

Profhilo

Profhilo is high-concentration hyaluronic acid injected at fixed points. It is chosen when thin, dehydrated skin adds to the sag.

Sculptra

Sculptra is a PLLA collagen stimulator that rebuilds volume over months. It is chosen when lost volume drives the sag and you can commit to a series.

Combination plans

Because sagging happens at several depths, combinations look most natural: ultrasound for the deep layer plus RF for the skin, as in our Seoul Signature Lifting package. Read why Thermage and Ulthera are combined or our guide to Korean lifting treatments.

At-home care and prevention

Wear SPF 50 daily, use a retinoid at night if tolerated (pause it a few days before treatment), avoid smoking and keep your weight stable.

Planning treatment during a trip to Seoul

Device lifting has little or no downtime, so you can sightsee the same day. Schedule threads and Sculptra early in your stay. Results build over two to three months, after you are home. Book a consultation and your doctor will confirm a plan that fits your dates.

When sagging needs a doctor's attention

Sudden drooping on one side of the face, especially with weakness or slurred speech, is an emergency; call 119 in Korea. RF is not suitable with a pacemaker, and lifting is postponed during pregnancy. See who should avoid or delay lifting treatments.

FAQ

Frequently asked questions

Straight answers from our medical team. Still unsure? Ask a doctor or browse all patient questions .

At what age does skin start to sag?

Collagen production starts to slow in your late 20s, and many people notice a softer jawline in their mid-30s to 40s. Genetics, sun exposure and weight changes move this earlier or later. Our guide on when to start lifting treatments explains the timing.

Can non-surgical treatments fix sagging skin?

They can visibly improve mild to moderate sagging by lifting the deep layer and tightening the skin. They do not match the result of a surgical facelift for heavy, advanced laxity. Your doctor will tell you which group you fall into.

Which lasts longer, Ulthera or Thermage?

Both commonly last around a year or longer, and Thermage results often hold for 12–18 months. They work at different depths, which is why they are often combined rather than chosen against each other.

How many sessions will I need?

Ulthera Prime and Thermage FLX are usually single sessions repeated yearly. Korean HIFU and RF devices are often repeated every 3–12 months, and Sculptra is given as a short series.

Is there downtime after lifting?

Most devices leave mild redness or tenderness for a few hours to a day. A thread lift can cause swelling or bruising for 3–7 days, so plan it earlier in your trip.
