HIFU vs RF Lifting: Which Is Right for You?

Quick answer: HIFU (focused ultrasound, such as Ulthera Prime and Shurink Universe) delivers heat to precise points as deep as the SMAS layer, so it is best for lifting jowls, brow and jawline. RF (radiofrequency, such as Thermage FLX, Oligio X and Volnewmer) heats the dermis broadly, so it is best for tightening crepey skin and fine lines. At Dernia Clinic Myeongdong, HIFU starts from ₩149,000 and RF from ₩390,000.

  • Core difference: HIFU lifts deep support tissue; RF tightens the skin itself
  • HIFU devices: Ulthera Prime (from ₩390,000, 100 lines); Shurink Universe (from ₩149,000, 300 shots)
  • RF devices: Thermage FLX (from ₩990,000); Oligio X and Volnewmer (from ₩390,000), all for 300 shots
  • Downtime: None to minimal for both
  • Results last: About 3–12+ months (HIFU); about 6–18 months (RF), depending on device
  • Combine? Yes, often on the same day, ultrasound first

HIFU vs RF lifting at a glance

Most non-surgical lifting in Korea uses one of two energy types: high-intensity focused ultrasound (HIFU) or radiofrequency (RF). Both heat tissue under an intact skin surface to tighten collagen and trigger new collagen, but they deliver that heat in very different patterns and at different depths.

Feature HIFU (focused ultrasound) RF (radiofrequency)
Technology Ultrasound waves focused to a point, like light through a magnifying glass Electrical current that meets the tissue's natural resistance and turns into heat
How it works / target depth Tiny thermal points at fixed depths, about 1.5 to 4.5 mm, reaching the SMAS Broad, even heating of the dermis and the fibrous bands beneath it
Best for Jowls, brow, jawline and under-chin: lifting Crepey skin, fine lines, eyelids and overall firmness: tightening
Comfort Sharp, deep heat at each point, strongest over bone Waves of heat, softened by surface cooling
Downtime None to mild swelling or tenderness None to minimal redness
Sessions Usually one, repeated every 3–12 months by device Usually one, repeated every 6–18 months by device
Results appear Gradual over 1–3 months Some firming early; builds over 2–6 months
Results last About 3–6 months (Shurink) to 12 months or longer (Ulthera) About 6–12 months (Oligio X, Volnewmer) to 12–18 months (Thermage)
Price from at Dernia Shurink Universe ₩149,000 (300 shots); Ulthera Prime ₩390,000 (100 lines) Oligio X and Volnewmer ₩390,000; Thermage FLX ₩990,000 (300 shots each)

How HIFU works

HIFU focuses ultrasound energy so it converges at a set depth below the skin. At that focal point, tissue briefly heats to a coagulation temperature, creating a tiny thermal point while the layers above stay intact. Rows of these points are placed at chosen depths, including 4.5 mm, which reaches the SMAS: the muscle and connective-tissue layer a surgeon tightens in a facelift. Over two to three months the points contract and remodel, giving a gradual lift. Ulthera Prime adds real-time imaging so the doctor can see each layer, while Shurink Universe uses fixed-depth cartridges for lighter, more frequent sessions.

How RF works

RF passes an electrical current through the skin. Tissue resists that current and the resistance produces heat, spread across a volume of the dermis rather than concentrated in points. A cooled tip protects the surface while the deeper skin warms enough for collagen to contract and rebuild. The effect is tighter, smoother, firmer skin, especially where it has become thin and crepey. Thermage FLX is the original monopolar RF system, and Korean devices such as Oligio X and Volnewmer use the same principle at a lower price.

Choose HIFU if…

  • Your main change is descent: early jowls, a lower brow or fullness under the chin
  • You want a sharper jawline rather than smoother skin
  • You have a moderate amount of facial fat and thicker skin
  • You want to target the SMAS layer without surgery

Choose RF if…

  • Your skin is thin, crepey or finely lined
  • You want to treat the eyelids or the delicate skin around the eyes
  • Your face is slim and you want to avoid focused heat in the fat layer
  • Enlarged-looking pores and texture are part of your concern

Can you combine them?

Yes, and combining is common in Korea because the two work at different depths. HIFU lifts the foundation and RF tightens the skin draped over it, much like tightening a tent frame before smoothing the canvas. Many doctors perform both on the same day, usually ultrasound first. Popular pairings include Ulthera with Thermage and Shurink with Volnewmer. See our guide to combining Thermage and Ulthera and the detailed Thermage vs Ulthera comparison.

How our doctors decide

Your doctor assesses where the change comes from: loose skin, descending deeper tissue, fat or a mix. Skin thickness, facial fat, bone structure, age and how often you can return to Seoul all shape the plan. Many doctors are cautious with HIFU in very slim faces, because focused heat in the fat layer can occasionally lead to a hollow look.

RF is not used if you have a pacemaker or other implanted electronic device, and neither is done during pregnancy or over active infection. Learn more in our guide to Korean lifting treatments and our page on sagging skin, or book a consultation and a licensed doctor will tell you whether HIFU, RF or both suit your face.

FAQ

Frequently asked questions

Straight answers from our medical team. Still unsure? Ask a doctor or browse all patient questions .

Is HIFU or RF better for sagging?

For sagging caused by deeper tissue descent, such as jowls, HIFU is usually more effective because it reaches the SMAS layer. For loose, crepey skin, RF usually does more. Many faces have both, which is why the two are often combined.

Which hurts more, HIFU or RF?

HIFU tends to feel sharper and more intense, particularly over bone, because energy is concentrated in points. RF feels like waves of heat softened by cooling. Numbing cream is used for both, and energy is adjusted to your comfort.

Is Ulthera HIFU?

Ulthera uses microfocused ultrasound with real-time imaging, a form of focused ultrasound in the same family as HIFU. Shurink Universe is a Korean HIFU device without imaging. Both lift from the same depths.

Can HIFU make my face look thinner?

HIFU can slightly reduce fullness in some areas, which some people want for a V-line. In very slim faces, it can occasionally cause a hollow look, so your doctor adjusts depths and energy or may suggest RF instead.

How often should I repeat HIFU or RF?

It depends on the device: Shurink every 3–6 months, Ulthera about once a year, Korean RF every 6–12 months and Thermage every 12–18 months. Your doctor will suggest a schedule that fits your skin and travel plans.
