What causes forehead wrinkles?

Horizontal forehead lines come from the frontalis, the muscle that raises your eyebrows. Each time you look surprised, concentrate or lift your brows, the skin above folds. When you are young, it springs back. After years of movement, collagen loss and sun damage, the folds start to stay. Some people raise their brows constantly without noticing, often to lift heavy or hooded eyelids, which is why forehead lines can appear in the late 20s. Very expressive people, those who squint without sunglasses and people with thinner skin tend to develop lines sooner.

How forehead wrinkles are assessed

At Dernia Clinic Myeongdong, your doctor watches your forehead at rest and as you raise your brows. Lines fall into two types:

  • Dynamic lines: appear only with movement and respond very well to Botox
  • Static lines: stay visible at rest because the skin has creased, so they need Botox plus treatments that improve the skin

Your doctor also checks your brow position and eyelids. If your forehead muscle is helping hold up heavy lids, relaxing it too much can make the brows feel heavy, so the dose is planned carefully. Your doctor may also ask you to frown and squint, since frown lines and crow's feet are often treated in the same visit.

Treatment options for forehead wrinkles

Wrinkle Botox

Wrinkle Botox relaxes the frontalis so the skin stops folding. It is the first choice for dynamic lines and the base of most plans. You can choose a Korean toxin such as Botulax, Nabota or Coretox, or an imported brand such as Botox, Xeomin or Dysport. Korean toxins cost less, while some people prefer a brand they know from home. Both work well when dosed correctly.

Skin Botox

Skin Botox places tiny amounts of diluted toxin just under the skin surface. Your doctor chooses it for fine, crepey forehead texture or oiliness when you want to keep more natural movement. It is often placed across the forehead and cheeks in one session and lasts about 2–3 months.

Hyaluronic Acid Skin Booster

A hyaluronic acid skin booster hydrates the skin from within so etched static lines look shallower. It is chosen alongside Botox when lines remain at rest, and it adds glow. Results last about 1–3 months, so it also works as a top-up before an event.

Combination plans

For most people, Botox alone is enough for dynamic lines. When lines are etched, a typical plan is Botox first, then a skin booster once the muscle has relaxed. For early lines, our prevention package for your 30s combines skin Botox, Oligio X and an HA booster.

At-home care and prevention

Use SPF every day, wear sunglasses to reduce squinting, and use a retinoid at night if your skin tolerates it. Keep the skin well hydrated. Treating dynamic lines early can help stop them from becoming etched.

Planning treatment during a trip to Seoul

Botox takes 10–15 minutes with no downtime, so it fits any itinerary. It starts working in a few days and peaks at around two weeks, so treat early in your trip if you want your doctor to check the result. For a few hours afterward, avoid rubbing the area, lying flat or intense exercise, as your doctor advises. Flying is usually fine; see can I fly after Botox. Book a consultation to plan your dose.

Safety and when to see a doctor

Botox is not given during pregnancy or breastfeeding, or to people with certain neuromuscular conditions such as myasthenia gravis. Tell your doctor about any medicines you take. A heavy brow or slight eyelid droop after treatment is uncommon and temporary. Sudden one-sided facial weakness unrelated to treatment is a medical emergency. FAQ

Frequently asked questions

Straight answers from our medical team. Still unsure? Ask a doctor or browse all patient questions .

How many units of Botox does the forehead need?

It depends on your muscle strength and the brand, since units are not interchangeable between all toxins. Your doctor sets a conservative dose to keep natural movement and avoid heavy brows.

Is Korean Botox as good as imported Botox?

Korean toxins such as Botulax and Nabota are approved by Korea's Ministry of Food and Drug Safety and widely used. Some people prefer imported brands; read is Korean Botox safe to compare.

How long does forehead Botox last?

Usually about 3–4 months. Many people repeat it two to four times a year, and lines often look softer over time as the habit of frowning or brow raising eases.

Will I look frozen?

Not if the dose is matched to you. The aim is to soften lines while keeping some natural movement, and your doctor can fine-tune the amount at your next visit.

Can Botox fix deep forehead lines?

Botox stops lines from getting deeper and softens them, but deeply etched creases improve gradually. Adding a skin booster helps them look shallower.
