What is under-eye filler?

Under-eye filler, also called tear trough filler, uses a small amount of soft hyaluronic acid (HA) to fill the hollow groove between the lower eyelid and the cheek. That groove casts a shadow that makes you look tired even after a good night's sleep, and softening it gives a smoother, more rested look. Because the skin here is thin and the anatomy delicate, this is one of the most technique-sensitive filler areas. At Dernia Clinic Myeongdong, under-eye filler starts from ₩290,000 per 1cc and is performed only by licensed medical doctors.

How under-eye filler works

A soft, low-swelling HA is placed deep, close to the bone, usually with a blunt cannula and in small amounts. It supports the hollow from below so the shadow lifts. Most people need well under 1cc for both sides. More product does not mean a better result here: excess filler can look puffy or show through thin skin as a bluish tint, known as the Tyndall effect. Sometimes a little support in the upper cheek softens the lid-cheek junction better than filling the groove directly, and your doctor may suggest this instead.

Who it suits, and who it does not

  • Good fit: a visible groove under the eyes with reasonably firm skin (under-eye hollows)
  • Good fit: shadow-type dark circles caused by the hollow rather than pigment
  • Less suitable: prominent fat bags, which filler can make look larger
  • Less suitable: brown pigmentation or visible veins, which filler does not change
  • Less suitable: frequent morning puffiness or fluid swelling, which HA can worsen

Your doctor examines you sitting upright in good light, looking at bone, fat and skin, before recommending filler. If under-eye bags are the main issue, Agnes RF may be considered instead.

Your session at Dernia, step by step

  1. Consultation in English — your doctor assesses the hollow, skin thickness, fat bags and pigment, and tells you honestly whether filler is right for you.
  2. Numbing — topical cream, plus a little local anesthetic at the cannula entry point.
  3. Injection — tiny amounts placed close to the bone and assessed after each step with you sitting up.
  4. Review — a cold pack and a mirror check before you leave.

Downtime and aftercare

Mild swelling and possible bruising last two to five days. Use a cold pack gently, sleep with your head slightly raised for two nights, and avoid rubbing your eyes, alcohol, saunas and hard exercise for 48 hours. Concealer can usually be used the next day. Go easy on salty food, which can increase puffiness.

Results timeline

The hollow looks softer straight away. Results settle at about two weeks, when any small top-up is considered. Under-eye filler usually lasts about 9–12 months, sometimes longer, because the area moves less than the lips. Avoid topping up too often: layering filler here every few months can build up into puffiness over time. See the under-eye filler results guide and our under-eye filler review.

Planning under-eye filler around a trip to Seoul

Allow at least five days before you fly or attend an event, so any bruising can settle and your doctor can check the result. Flying itself is fine, though long flights can make the area slightly puffy. If thin, crepey skin is part of the picture, the Eye Rejuvenation Package combines Rejuran Eye, Botox and Oligio X. Book a consultation and your doctor will assess your eyes first.

Safety and who should avoid it

The under-eye area sits near blood vessels connected to the eye, so vascular occlusion, while rare, is the most serious risk and can, in very rare cases, affect vision. Your doctor reduces it with a blunt cannula, deep placement, slow small-volume injection and hyaluronidase on hand. Contact the clinic immediately if you notice worsening pain, skin color changes or any change in your vision. Other possible issues include lumps, a bluish tint or prolonged swelling, which can be corrected by dissolving the filler. It is not recommended during pregnancy or breastfeeding, or with an eye or skin infection, a bleeding disorder or marked fluid puffiness. Browse our other injectable treatments.

FAQ

Frequently asked questions

Straight answers from our medical team. Still unsure? Ask a doctor or browse all patient questions .

Is under-eye filler safe?

In experienced hands it is a well-established treatment, but it is one of the more delicate filler areas. Doctors use a blunt cannula, small volumes and deep placement to reduce the risk of complications, and HA can be dissolved if needed.

Will under-eye filler fix my dark circles?

Only if the darkness is a shadow caused by a hollow. Brown pigment, visible veins or thin skin need different treatments, such as laser toning or Rejuran Eye, which your doctor can explain.

Why do my eyes look puffy after filler?

Some swelling in the first few days is normal. Puffiness that lasts beyond two weeks can mean the filler is holding fluid or there is slightly too much, and your doctor can adjust or dissolve it.

How much filler do I need under my eyes?

Usually less than 1cc in total for both sides. Your doctor may add a little more at a two-week review rather than placing it all at once.

Does under-eye filler hurt?

With numbing cream and a cannula, most people feel pressure and a little stinging at the entry point rather than pain. The injection part takes only a few minutes.

Can I wear makeup afterward?

You can usually apply concealer gently from the next day. Avoid pressing or rubbing the area for the first few days.
